E-FORMS FOR NEW PEDIATRIC DENTAL PATIENTS

Coastal Kids Dental & Braces values your time and we work hard to stay on schedule with every child's dental appointment.  To help us be as efficient as possible please visit our HIPPA compliant secure server to complete your child's Health History and Insurance information prior to your child's first dentist visit.

This enables Coastal Kids Dental & Braces to provide a 100% paperless dental practice as we do our part to GO GREEN. 

You DO NOT need to print the E-Forms to bring in with you for your child's dental appointment.  Once your information is submitted it is sent directly to our server.  When you visit our office for your appointment you will be given the opportunity to review your answers on our lobby Kiosk computer to make any changes needed or approve as originally submitted online. 

Please complete one E-Form for each child who has an dentist appointment.
